Service Description: Access to health care support at home, at school or in the community
  • Care Coordinators work with people of all ages in schools, in their homes and community, and support the safe transition home from a hospital stay and arrange for care supports
  • Care is provided in community settings such as a home, school, residential facility or CCAC Community Nursing Clinic
  • Information and referral to other community services when appropriate * provided through the information and referral telephone line in English or French * or online access to health care information and services in Ontario at www.thehealthline.ca

  • Health Care Connect (for individuals who a looking for a doctor)
  • In-Home Services
  • Mental Health and Addictions School Program
  • Nursing Services in a CCAC Community Nursing Clinic
  • Telehomecare Program

Specialized services may include:
  • Access to assisted living and supportive housing
  • Access to respite care and convalescent care (short-term stays and adult day programs)
  • Access to Long-Term Care Homes, rehabilitation and complex continuing care beds
  • School health support services to school aged children
  • Community support for people living with cancer
  • Hospice palliative care

Fees: None - CCAC services are covered by OHIP
