This symposium will focus on the ultimate goal of providing information regarding excellence in stroke care across the continuum of care from the most recent research and emerging evidence in areas of This symposium will focus on the ultimate goal of providing information regarding excellence in stroke care across the continuum of care from the most recent research and emerging evidence in areas of health promotion, secondary prevention, acute stroke care, acute rehab and post- acute rehabilitation while maximizing community integration and participation. The objective is to bring Best Practices Evidence and Research to care providers in Central East Stroke Region which includes NSMLHIN, CLHIN and CELHIN. The planning committee has strong representation from across the region. The target audience includes all health care providers involved with Stroke Care from across the Continuum of Care, including Prevention, Acute Care to Rehabilitation, and Rehabilitation to Reintegration into the Community or LTC placement. Target Audience: Physicians/ Nurse Practitioners/ Family Health Teams Therapy Providers (OT, PT, SLP, SW, RecT, Nurses, & Health Care Administrators.
