Service Description: Expert nurses providing care through a teaching model, services include:
  • wound care
  • IV medication administration and teaching
  • PICC lines, Portacath, Chemo disconnects
  • ABIs
  • Health Teaching
  • Injections
  • Symptom management
  • Ostomy care and teaching
  • Catheter care, Nephrostomy care, Drain care, G Tube care
  • Physical Assessments

All patients attending the community nursing clinic will be assessed and provided with individualized instruction on self care and self-management supports

Fees: None - Services paid for by OHIP
Application: By referral only * patient will be contacted by St Elizabeth with appointment details * patients can schedule an appointment at times that is convenient for them
