Community-based program that instills goals and objectives for those who have a diagnosed, serious mental illness * focus on recovery and wellness
Services and initiatives:
- assistance with daily living activities
- brief and long term support
- life skills development
- coping strategy development support
- employment/vocational and social activities support
- navigation of the mental health system
- referrals to other community resources and coordination of support
- symptom and medication management
- crisis prevention, intervention and management
- health promotion and teaching
