Service Description:

Care for those affected by or living with the effects of a brain injury * partnership between Mackenzie Health's Centre for Behaviour Health Sciences and March of Dimes Canada
Services include:- case coordination and comprehensive functional assessment to evaluate the effects of the acquired brain injury on the individual and those who support them
- individualized service/treatment plan, developed with recommendations and community support to promote the acquisitions of skills and strategies
- plan which will enable persons with an acquired brain injury to live with maximal fulfillment in a state of dignity within the community
