Service Description: Support for people living with diabetes, pre-diabetes, Type 1 diabetes
  • one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise and other lifestyle choices
  • registered nurse, dietitian on staff

Additional services available in group and individual settings include:
  • starting insulin
  • insulin pumps
  • education programs
  • pregnancy and diabetes
  • education awareness events
  • education and training for health care professionals
  • community service group

Fees: Free
Application: Central referral from Adult Diabetes Central Referral Program * physician referral * health worker referral * self-referral
Eligibility / Target Population: Adults and older adults with diabetes (Type 1 and Type 2)
