Service Description: Services that guide those with physical disabilities to independency * self-directed services * personal support and homemaking services are available * outreach services can be customized to fit the environment that is needed: home, place of employment or educational institution
  • meal preparation
  • housekeeping
  • assistance with shopping, banking and appointments
  • involvement in scheduling outreach services
  • nurturing services for parenting assistance
  • assistance with mobility

If an individual requires the assistance of a Power of Attorney (POA) or Substitute Decision Maker (SDM) that individual will not qualify for this self-directed program

Fees: Free * No fee for attendant services to eligible applicants
Application: Self Referrals * referral form can be downloaded from website * Call for more information
Eligibility / Target Population: Youth, young adults and adults that have a permanent disability that requires physical assistance with daily activities * ability to direct own care * must be 16 years and older
