Services that guide those with physical disabilities to independency * self-directed services * personal support and homemaking services are available * outreach services can be customized to fit the environment that is needed: home, place of employment or educational institution
- meal preparation
- housekeeping
- assistance with shopping, banking and appointments
- involvement in scheduling outreach services
- nurturing services for parenting assistance
- assistance with mobility
If an individual requires the assistance of a Power of Attorney (POA) or Substitute Decision Maker (SDM) that individual will not qualify for this self-directed program
