Service Description:
|
Supports adults with developmental disabilities who are living independently or planning to live independently within their chosen community * designed to support through leading a life reflective of personal values and choices and assisting them in achieving individualized goals such as seeking accommodation, budget counselling, short-term emotional support, or respite care, among others
Program includes:- individualized planning based on the goals and support needs of the adult
- ongoing case management
- assistance in accessing and coordinating all necessary and available community services

Eligibility / Target Population:

Adults 18 years and older with developmental disabilities and are living independently or planning to move towards independence in a community setting without significant social support are eligible for this program
