Service Description:

Patients provided with services that address specific health-care needs, such as IV therapy, wound care, or rehabilitation therapy * community clinics are staffed by specially trained nurses (not the same as visiting a walk-in medical clinic)

No ‘walk-in’ appointments * All visits are scheduled in advance at a time convenient to the patient by a care coordinator

Eligibility / Target Population:

All eligible patients (adults and youth) requiring Home and Community Care nursing services will receive care at a community nursing clinic
