Position: Manager, Community Health and Wellbeing

Description: Job overview:
Reporting to the Director, Clinical and Allied Health Services, the CHW Manager will
provide leadership, strategic direction, and operational oversight for programs that support
primary care providers and enhance access to integrated, patient-centered care. This role
is ideal for a collaborative, equity-focused leader with strong experience in healthcare
program management, community partnerships, and team development.
Qualifications:
Education & Experience
- Bachelor's degree in Health Administration, Health Promotion, Allied Health, Evaluation, or a related health profession.
- Minimum 3 years of experience in community program delivery, preferably within healthcare or social services.
- Minimum 5 years of progressive leadership, supervisory, or management experience, including program development, evaluation, and budget management.
Knowledge & Skills
- Experience leading interdisciplinary healthcare or community-based teams.
- Knowledge of primary care delivery models, health promotion, and community development principles.
- Strong communication, relationship-building, organizational, and problem-solving skills.
- Experience with quality improvement methodologies and program evaluation.
- Understanding of equity, diversity, inclusion, and anti-oppression principles.
- Knowledge of/experience working with First Nations, Métis, and Inuit and/or Francophone individuals and communities is preferred.
- Bilingualism (French and English) and/ or Indigenous self-identification will be considered an asset.
Responsibilities:
- Lead and support an interprofessional team delivering primary care community support programs.
- Build and maintain strong relationships with primary care providers, community partners, and the North Simcoe Ontario Health Team.
- Oversee program planning, implementation, evaluation, quality improvement, and reporting.
- Champion community engagement, health equity, and patient-centered care.
- Support organizational and community-wide strategic priorities through innovative and responsive programming.
Employment Requirements:
- Criminal Record Check with Vulnerable Sector Screening.
- Valid Ontario Driver's Licence and access to a reliable vehicle.
- Ability to work occasional evenings and flexible hours as required.
Salary: $97,500.00-$103,350.00
