Keep Me Home Program - Keep Me Home supports individuals faced with a life threatening illness who wish to die at home and their families. Keep Me Home is based on the concept that the people in our lives, friends, families, neighbours, coworkers and members of our faith communities wish to help. Very often, family and friends want to help but don’t know what to do. Keep Me Home creates a caregiving team that meets the unique needs of each family at-end-of-life. There is no fee for this program.
