Service Description:

Patient-centred education and support to help people living with Diabetes or Prediabetes to manage their disease
Services include:
- one-on-one sessions with registered dietitians and registered nurses (certified diabetes educators)
- ongoing information and education
- insulin pump starts and support
- endocrinology clinics (referral to the Endocrinologist is required)
- group programming which includes activities and cooking
