Service Description: Support for people living with pre-diabetes or diabetes * Support for pregnancy with type 1 or type 2 diabetes and gestational diabetes * one-on-one or group consultation on monitoring blood sugar levels, healthy eating, how to administer insulin and use insulin pumps * registered nurse, registered dietitian, Certified Diabetic Educator (CDE) on staff

Education and awareness seminars and workshops are offered to the general public in the community

Application: Central referral from Adult Diabetes Central Referral Program * referral from physician, registered nurse, health worker referral * self-referral * form available on website
Eligibility / Target Population: Adults and older adults with diabetes (Type 1 and Type 2) and gestational diabetes
